Mental Health Case Management
Hope for children and adults with emotional disturbances

Mental Health Case Management helps navigate and coordinate services to assist in improving the functioning and stability of children and adults who have mental health concerns. Case Managers work in direct collaboration with families and adults to access services that need their specific needs. Services include mental health, social education, vocation, recreation, transportation and advocacy.

ParticipantsChildren with severe emotional disturbances (SED) and adults with Severe and Persistent Mental Illness (SPMI) who live in Hennepin (children only), Stearns, Benton, Mille Lacs, Wright, Isanti, and Chisago Counties counties |
FeesAll prepaid Medical Assistance providers (PMAP) are accepted and include Health Partners, Medica, UCare, Blue Plus, and Hennepin Health In Hennepin County, we accept Medical Assistance, county-paid for uninsured or private insured, or PMAP providers In Benton, Chisago, Sterns, Wright, Isanti, and Mille Lacs counties we accept all PMAP providers. |
How we help
Each participant’s family is assigned a case manager who assists them in determining what services are needed, helps the family obtain the services, assesses new needs, and coordinates with all service providers who work with the family to keep the services working effectively.
